Licensed Vocational Nurse II
The Denton State Supported Living Center (SSLC) is seeking a Licensed Vocational
Nurse (LVN) II-Specialty Clinic Nurse to join an interdisciplinary team of professionals to provide clinical care to individuals of the SSLC. In this position you will work at the SSLC campus with a dynamic group of staff and professionals who are
dedicated to providing supports and services for individuals with intellectual and developmental

The Specialty Clinic Nurse performs highly complex vocational nursing work, delivering the nursing care necessary to meet the health care needs of
individuals who live at the Denton SSLC. Performs focused assessments and care intervention,
such as health screenings. Evaluates and documents healthcare needs and/or issues and reports health care needs and/or issues to the appropriate person(s).
Responsible for the management of the clinic. Works directly with the consultant physician/Primary Care Provider (PCP) and closely with clinic and nursing staff to ensure pertinent results for their assigned clinics are entered into the database.
Communicates with others, such as direct care staff, physicians/PCP, administration,
and other nursing staff to ensure optimal care as part of the interdisciplinary team.
Notifies the unit clinic nurse and/or Registered Nurse Case Manager (RN-CM) of trends and concerns to ensure that communication is in place to provide consistent, high quality care for all persons served. Maintains a professional, harmonious working relationship with facility employees as well as outside agency employees.
Keeps informed of and follows all agency, facility, and unit policies and procedures.
Assists the Oversight RN with on-the-job nursing instruction and training of other new LVNs as
instructed. May supervise the work of others. Completes other assignments as instructed. Works under the general supervision of the Oversight Registered Nurse or other designated supervisor, with moderate latitude for the use of initiative and independent judgment.

Essential Job Functions:

Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift, and/or overtime in accordance with agency leave policy and performs other duties as assigned.

Organizes, prepares and processes all paperwork for assigned clinics including but not limited to paper consults, billing sheets, scheduling in SCHAPPT book and/ or via established methods, etc.
Documents and reports observable descriptors of actions and behaviors during consultant visits to provide information to PCPs and teams to guide treatment decisions. Completes notes for missed appointments in IRIS (EHR); documents no-call/no shows in notes. Documents according to facility policy.

Performs focused assessments and nursing procedures skillfully and thoroughly in accordance with training and facility guidelines to assure delivery of appropriate and optimal nursing care. Demonstrates thorough knowledge of the nursing process and nursing procedures.

Participates in Quality Assurance, Peer Review and other activities related to the development/enhancement/refinement of nursing policy and procedure in order to evaluate and improve the quality of nursing care provided on campus.

Follows Denton SSLC guidelines in regards to established nursing guidelines, appropriate use of forms and processes. Documents assessments and findings as per facility policy/guidelines.

Assists physicians in on campus clinic setting and PCPs in off campus clinic setting, recognizing and reporting signs and symptoms of illness/condition, and reports accurately to RN and/or Primary Care Provider (PCP). Collects data for RN/PCP.

Keeps clinic area clean and organized. Ensures that proper supplies are available in clinic and orders needed equipment from Central Supply.

Observes infection prevention principles by following standard precautions to minimize cross contamination.

Administers medications and treatments orally, rectally, or by injection in case of emergency.

Serves as resource/mentor to LVNs. Cross-trains floor nurse(s) to back-up in the clinic, as needed. May supervise others, if needed.

Assists in lab to draw blood samples as per scheduled time periods, etc. Collects specimens for laboratory examination and conducts appropriate screening tests (after competency is documented) as requested by lab personnel.

Provide treatment to HHS employees injured in course and scope of employment

Performs technical Nursing procedures according to facility and state regulations after demonstration for competency, i.e., tracheostomy care, gastrostomy/jejunostomy care, wound care, breathing treatments. Maintains competency in use of all medical equipment as ordered by physician/PCP.

Responds to emergency situations to ensure safety of individuals. Performs CPR, suctioning, and other emergency procedures as directed by physician/PCP/protocol. Administers emergency medications as prescribed. Follows safety and Infection Control Procedures of facility.

Demonstrates compliance with time and attendance policy and assigned work hours in order to provide adequate nursing coverage in the assigned unit. Observe Standards of Conduct of State Officers and Employees as defined in HHSC HR Manual. Adheres to professional standards of conduct and behavior.

Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations Plan (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.

Knowledge Skills Abilities:

Considerable knowledge of nursing procedures, techniques, principles, and processes used in providing individual care.
-Knowledge of anatomy, physiology, and pathophysiology, nursing techniques and procedures, medications and side effects, adverse reactions and signs of toxicity associated with medications.
-Knowledge of, or ability to learn the techniques of, administering medications to developmentally disabled individuals.
-Knowledge of mathematics as it relates to nursing practices and procedures.
-Ability to communicate effectively, both orally and in writing, with a wide variety of people of varying skill level and medical knowledge to ensure optimal care for persons served.
-Ability to train others.
-Ability to apply principles and practices to do focused assessments, and to prevent and treat disease processes.
-Ability to utilize critical thinking skills to identify problems, assess the situation, and ensure resolution.
-Must maintain current CPR certification and have strength and flexibility to safely perform Cardiopulmonary Resuscitation (CPR).
-Ability to demonstrate a working knowledge of emergency procedures and emergency equipment.
-Ability to prioritize work and practice time management to ensure time lines are met.
-Ability to perform focused assessments and evaluate the healthcare needs and or issues of persons served.
-Skill and competency in use of all medical equipment.
-Skill in providing individual care, and preparing and maintaining records.

Registration or Licensure Requirements:

Licensed to practice as a licensed vocational nurse in the state of Texas (or a state that recognizes reciprocity through the Nurse Licensure Compact).

Initial Selection Criteria:

Graduation from an accredited nursing program is required.
-Experience in focused assessments and documentation of health status.
-Two (2) years nursing experience is preferred.
-Experience leading a team, training, or supervising the work of others is preferred.
-Experience using and electronic health record system is preferred.

Additional Information:

Applicants must pass a fingerprint criminal background check, pre-employment drug screen, and registry checks including the Client Abuse/Neglect Reporting System (CANRS), Nurse Aide, Medication Aide and Employee Misconduct and HHS OIG List of Excluded Individuals/ Entities. All state supported living center employees are subject to random drug testing. Flexibility in work hours may be required for this position. The position may be required to work overtime and/or extended hours. Must be willing to work weekends and holidays.
