Combination of remote work (2 days per week) and field work (3 days per week) Service / Care Coordinator PAY RATE: $25-$26 per hour based on experience plus mileage reimbursement Service / Care Coordinator HOURS: M-F 8:00am-5:00pm Central Time Service / Care Coordinator Required Skills: Must be either an LVN/LPN or have a Bachelor’s Degree and relevant experience 5 years of experience working with the aging, blind, or disabled population groups 2 years of Case management / Service Coordination experience Bilingual in Spanish is required Strong documentation skills Excellent computer skills High School Diploma or GED Service / Care Coordinator Preferred Skills: Medical-Long Term Services and Supports Background, Working with the STARPLUS population. MCO Medicaid STARPLUS or STAR Kids or STAR Health insurance knowledge Service Coordination, Case Management or Nursing experience preferred. Background in Managed Care Familiarity and experience working with Medicare or Medicaid plans
