The Case Manager will be responsible for a broad range of tasks and responsibility, which falls under this role. The Case Manage I is under the supervision of the Chief Social Worker. Ensures the effective care and treatment of the patients by providing a safe, therapeutic, Social Learning environment through demonstration, application, and supervision of other staff in the principles and techniques of the Social Learning Rehabilitation Program. Serves as an effective champion for the Social Learning approach. This individual will assume a lead patient advocate role and will coordinate functions to learning approach and will coordinate functions to ensure patients on the Social Learning Rehabilitation Program actively involve themselves in those activities which will promote acquisition of skills to enhance their ability to function more independently and successfully in the community upon resolution of legal conflict. Specific tasks are assigned relative to the program service delivery and each Case Manager will possess sufficient knowledge and skills to provide adequate services related to the assessment and treatment of forensic mental health patients and to assist and consult with staff representing other disciplines in preparation of appropriate recovery programs for each patient.

The Case Manager I will serve as case manager for assigned patients and be a member of the multidisciplinary treatment team. They will lead Competency Groups and consistently model socially acceptable interactions and behaviors. They will use positive reinforcement to facilitate desired outcomes. They will support and implement decisions made within the QIT and QIC.

Essential Job Functions:

Functions as an effective member of the Treatment / Recovery Team in the formulation, implementation, and review of comprehensive treatment plans for patients on the unit. Completed required documentation using information from pertinent clinical evaluation tools and processes maintained within the Social Learning Rehabilitation Program. Will coordinate patient advocate functions to include, but not limited to MHA visits, visitation lists acquisitions of social security cards, eyeglasses, etc Actively participates in all necessary audits, evaluations, and observations. Demonstrate problem-solving skills and promote a sense of teamwork by being an active participant in clinical management / patient care / in-service meetings. Conduct Competency Restoration groups. Knowledge Skills Abilities:

Assume the responsibility to act professionally on his / her judgment and initiates requests for professional support on a consultative basis as needed. Possess an ability to work cooperatively and effectively with the other program staff. Possess an ability to communicate effectively using both oral and written methods of communication. Possess a general knowledge of available community resources and the means to access them effectively. Demonstrates an ability to read and comprehend clinical assessments, identify pertinent information, and be able to integrate these reports and information into treatment plans. Associate degree from an accredited college or university with major course work in social, behavioral, or related field. Related work experience may substitute for the required education for a year for year basis with maximum substitution of two (2) years. Additional Information:
